New Patients: On the day of your appointment, please bring with you:
Please bring: your insurance card(s), photo ID, a list of current prescriptions with dosages, and any over-the-counter supplements you take. If you use a mail-order pharmacy, please also bring your drug formulary list.
Medical Records and Forms Completion Requests
We have partnered with MediCopy to fulfill Release of Information requests and disability/FMLA forms. MediCopy is fully HIPAA compliant and adheres to all state and federal regulations concerning release of medical information.

Please provide an email address on the authorization to expedite the process and delivery method. Records sent via email or to another provider are complimentary. Requests are processed within two (2) business days after receipt by MediCopy.

Disability/FMLA Forms
To submit FMLA/Disability forms, please visit https://medicopy.net/forms
A form-processing fee of $35 for the first form and $15 for each additional form submitted at the same time applies. Once payment is received, completed FMLA/Disability forms will be available within three (3) business days.
